Southern Utah Dietitian Conference Group Discount
How to Register a Group:
- Registration: Enter all registrant information on this form. Please do not use any forms from previous years. The grand total will calculate as you complete the form, including any applicable group discounts that may apply. Fill the form out completely for each attendee before submitting.
- Submission: Email the form to eatrightutah@live.com and describe the group that is being registered.
- Payment: Submit payment through UAND's payment webpage. Follow instructions for payment by credit card or check.
Early Registration pricing will only be honored if payment is received by the "Early Registration" deadline.
Register 10 or more dietitians to receive 20% off total registration!
**Please note, because students already receive conference admission at a discounted rate, we are unable to extend the group rates to student registrations.
